I have a copy with a mis-press where side 2 is actually side 4 of Zen Arcade. Anyone heard of this before?

I assume this was pressed at Rainbo Records even though it doesn’t appear in the matrix? Did all of the stuff mastered at Greg Lee Processing get pressed at Rainbo?

Some early pressings supposedly came with an insert/poster, can anyone confirm or add on this? Mine looks just like the one on the release page but didn't come with a poster.
